If you're photographing your baby at home, clear the background of anything distracting. Be aware of clutter on the counter and try to keep the focus on the baby. Often frosting (especially colored frosting) will leave some residue, so be sure to consider the possibility of a baby being nearby and make sure anything the baby can touch is easy to clean.

Perhaps a favorite lovey or other item that baby likes to hold (these are super sweet, especially if they have sentimental value to the family). If baby is in a newborn class with me, I like to incorporate an element of the newborn class into the cake smash. Perhaps a lovey that they hold, or a bed that they lie in to show how much they have grown.
